Hospital Sub-Acute Affiliations
Our hospital affiliations include Saint Peter’s University Hospital, Robert Wood Johnson University Hospital (New Brunswick) and University Medical Center of Princeton. Dr. Baxi also performs rounds at sub-acute facilities including McCarrick Care Center, and Care One in East Brunswick.
Referrals are welcome and much appreciated. We accept Medicare and most managed care insurances.

Please note, copays are expected at the time services are provided. Patients are responsible by bringing a valid referral prior to the visit. The practice reserves the right not to see a patient without a valid referral from the primary physician.
We require a 24 hour notice if you cannot keep your appointment. The practice reserves the right to charge a $25 no show charge if we do not get adequate notice.

New Patient Registration Packet (click here to download PDF forms) You will save time in the office if you can print the New Patient Registration Packet and fill it out in advance. All new patients are required to provide this information to our office along with some form of personal identification (such as a driver's license) and the current insurance card. If you have any questions while filling these forms, please do the best you can and our friendly staff will assist you on the day of your visit.
Naimish Baxi MD FAAPMR
Dr. Naimish Baxi focuses his expertise in the subspecialty of sports medicine, spine care, pain management, and musculoskeletal medicine. Dr. Baxi received his medical degree from New Jersey Medical School (formerly an institute of the University of Medicine and Dentistry of New Jersey, and now a unit of Rutgers School of Biomedical and Health Sciences). He completed an internship at the Jersey Medical Center (under the Barnabas health system), and residency in Physical Medicine & Rehabilitation at Mount Sinai Hospital in New York. Following his residency, Dr. Baxi completed an Interventional Spine & Sports Medicine at Orthopedic and Spine Specialists (OSS) Health in Pennsylvania.
Dr. Baxi has been published many times in the field of Physical Medicine & Rehabilitation.
Dr. Baxi is a New Jersey native – growing up locally in East Brunswick – and he and his wife currently reside in New Brunswick. He is fluent Spanish and Gujarati. In his spare time Dr. Baxi remains dedicated to wellness and staying physically active, and he has a passion for exercise and nutrition. He follows the Paleo diet and is currently a CrossFit Level 1 trainer (click to learn more).
Afreen Quraishi PA-C
Afreen Quraishi has a Master’s of Science and Certification in Physician Assistant studies from Towson University/CCBC Essex in Baltimore, Maryland. She did her rotations in multiple specialties such as Internal Medicine, Emergency Medicine, Obstetrics and Gynecology, but concentrated on Interventional Pain Management and Orthopedics. She joined Dr. Baxi in August 2017 after graduation.
Afreen is originally from Bel Air, Maryland and moved to New Jersey after getting married. She is fluent in Urdu and Hindi. Her hobbies include spending time with the family and friends, traveling, watching the NBA and TV in her spare time.
