Chronic Care Management (CCM)

With the guidance received through the CCM program, we hope to empower individuals with multiple chronic illnesses to understand and take control of their health, increase their quality of life and prevent hospitalization. Our clinician creates a comprehensive care plan for each individual that takes into consideration health conditions and specific goals to manage these chronic conditions. Other benefits include: medication reconciliation and refills, monthly check ins, the ability to have any medical questions answered as they arise, review of blood pressure measurements, weight changes and lab results as needed.

Nutrition Counseling

Appointments for nutrition counseling with a Registered Dietitian are covered through Medicare for those who have a diagnosis of Diabetes or Renal Disease. Good nutrition isn’t just important for maintaining your weight (although that’s certainly essential), it can also help lower your risk of other chronic diseases and boost your energy levels. Whether you want to lose weight, manage your chronic health condition, or simply maintain your well-being, the skilled registered dieticians at CMI can help! Diabetes Prevention Program

Did you know 1 in 3 American adults are considered prediabetic? By delivering The Diabetes Prevention Program, an evidence-based lifestyle change program, we hope to do something about that staggering statistic. This program is proven to prevent or delay type 2 diabetes by offering; a full year of support and instruction on how to eat healthy, add physical activity to your routine, manage stress, stay motivated, and solve problems that can get in the way of your goals. Participants who lost 5-7% of their body weight and added 150 minutes of exercise per week cut their risk of developing type 2 diabetes by up to 58% (71% for people over 60 years old)!

Obesity Counseling

A year-long program where participants receive weight loss counseling from a multi-disciplinary team consisting of a registered dietitian, exercise physiologist and a health coach. These weight loss counseling sessions can either take place in a group or one on one setting to help you achieve your health and wellness goals.
